Forms for Cremation

There are four options below to complete the cremation documents.
Please be aware in certain circumstances we may need multiple signatures for consent to cremate per Michigan law, for example if there are multiple children or siblings.
If multiple people need to sign and you elect to do it electronically, please have each person sign the electronic copy separately on their own computer, phone or tablet.
