Cremation Forms

Per Michigan law, multiple people MAY need to sign our documents to authorize release, cremation and receipt of the cremated remains.
This applies to the deceased having children, siblings, or parents.
If you select option 1, if multiple people need to sign, all individuals need to complete the packet separately.
